It’s about disqualification criteria that can end an astronaut’s career before it starts. Because once you’re three hundred million miles from Earth, you cannot call a therapist. You cannot take a break. And you definitely cannot quit.
The current medical standards for NASA astronauts are already brutal. You fail a vision test, you’re out. Bad back? Out. Kidney stones? Goodbye. But the mental health side is just as ruthless, and for good reason. Prospective crew members go through psychiatric interviews, psychological testing, and behavioral observation that would make a Navy SEAL selection look like a community picnic. They look for personality disorders, yes. But they also look for subtler things: excessive risk-taking, poor conflict resolution, inability to tolerate isolation. These aren’t quirks; they’re fatal mission liabilities.
Consider what a Mars crew actually faces. On a trip that takes six to nine months each way, plus a surface stay of a year or more, you’re looking at roughly three years of confinement with the same small group of people. No fresh air. No new faces. No private phone calls. No escape hatch. The International Space Station crews rotate every six months, and even they report tension, territorial behavior, and sleep disturbances. Multiply that by a factor of ten for deep space.
The disqualification criteria are designed to screen out people who might function fine in a normal life but crack under that pressure. A history of depression that’s well-managed on Earth? Probably disqualifying. Why? Because on Earth you have sun, exercise variety, social support, and medication refills. In space, your brain is bathed in cosmic radiation that may affect mood, you’re living under artificial light, and your medication supply is fixed. If that depression resurfaces 90 million miles from home, nobody can help. The mission becomes a rescue operation that doesn’t exist.
Same goes for anxiety disorders, even mild ones. Spaceflight involves constant micrometeoroid risk, system failures, and the knowledge that a single hull breach means death in seconds. A person who can compartmentalize that fear is fine. A person who fixates on it for six months is a danger to the crew.
The screening also looks for something called “psychological resilience under extended duress.” That’s a fancy term for not being an asshole when everything goes wrong. If you have a personality that needs external validation or control, you won’t pass. If you cannot accept that you might be wrong in a high-stakes argument with a crewmate whose oxygen supply you control, you’re disqualified. There are no exit interviews in deep space. There is no HR department. There is only the same three to six people and the vacuum outside.
This is why private companies like SpaceX and Axiom are already adapting NASA’s standards rather than inventing their own. They’ve realized the hard truth: you cannot train a personality. You can train a skill. You can teach someone to repair a CO2 scrubber. You cannot teach them to stop resenting a crewmate who snores in a capsule the size of a minivan for 200 consecutive nights.
The practical takeaway for anyone dreaming of a seat on a Mars-bound ship is blunt. Your technical skills matter, but your psychological profile matters more. A perfect mechanical engineer with a minor anxiety disorder will be passed over for a competent biologist with rock-solid mental health. That’s not unfair. That’s mission planning. Because the cost of a psychological breakdown in deep space isn’t just a ruined career—it’s potentially the end of the entire crew.
We’re headed to Mars. That’s not an if anymore; it’s a when. But the people who go will not be the bravest or the smartest. They will be the most stable. And the screening that filters them out will be the reason those crews come home.
